To avoid any misunderstandings regarding dental insurance, patients should be aware that all professional services rendered by our office are charged directly to the patient and that you are individually responsible for payment of all associated fees. Our office will be glad to submit any necessary forms or reports to help you obtain reimbursement from your insurance company. Our office does not render services on the basis that the insurance companies will pay for your treatment, in whole or in part. If your insurance information changes at any time, it is your responsibility to notify our office.
